During the Process Hazard Analysis that we are currently carrying out, there have been a number of occasions when we are referencing procedures and other documents to check how we are currently managing the various hazards.

It is always a bit worrying when you see that the document in question was written or last updated by yourself. So far there is nothing that I have found that I completely disagree with and am ashamed of now. But the worry is still there.

On a bright note, the fact that I would do some things differently shows that I have continued to learn and improve over the years.
